कैंसर की बायोप्सी रिपोर्ट मिलते ही “IHC”, “Grade”, “TNM”, “margin” या “biomarker” जैसे शब्द चिंता बढ़ा सकते हैं। इन्हें अलग-अलग समझने पर रिपोर्ट काफी स्पष्ट हो जाती है। कैंसर की बायोप्सी रिपोर्ट पढ़ने का आसान तरीका है चार सवाल पूछना कैंसर किस प्रकार का है, cells microscope में कैसे दिखते हैं, बीमारी कितनी फैली है और कौन-से biomarkers treatment चुनने में मदद कर सकते हैं। Pathology report diagnosis और treatment planning का महत्वपूर्ण आधार है, लेकिन किसी एक score से खुद treatment या outcome तय नहीं करना चाहिए।
कैंसर की बायोप्सी रिपोर्ट में सबसे पहले क्या देखें?
“Final Diagnosis” या “Histopathological Diagnosis” से शुरुआत करें। यह section tissue sample के आधार पर cancer का type और, कई मामलों में, subtype बताता है। इसके बाद Grade, lymph-node status, margin status और IHC या molecular tests देखें। College of American Pathologists के अनुसार pathology reports में tumor type, grade और कुछ molecular findings treatment planning के लिए उपयोगी हो सकती हैं।
Biopsy diagnosis में मदद करती है, लेकिन complete staging के लिए imaging और दूसरी clinical जानकारी भी जरूरी हो सकती है। Report के सभी sections को एक साथ पढ़ना बेहतर होता है।
Treatment discussion में डॉक्टर से कीमोथेरेपी जैसे विकल्प का उद्देश्य भी पूछें क्या इसे surgery से पहले, surgery के बाद, या cancer control के लिए दिया जा रहा है?
IHC क्या है और IHC Positive या Negative का क्या मतलब है?
IHC यानी Immunohistochemistry एक laboratory technique है, जिसमें antibodies की सहायता से tissue में खास proteins या markers पहचाने जाते हैं। Pathologist staining pattern से cancer की पहचान, subtype की पुष्टि या कुछ treatment-related markers का आकलन कर सकता है।
“IHC positive” का मतलब आम तौर पर संबंधित marker का detect होना है; इसका अर्थ अपने-आप “cancer ज्यादा गंभीर है” नहीं होता। इसका clinical meaning marker और cancer type पर निर्भर करता है।
Breast cancer में ER, PR और HER2 जैसे markers treatment planning में महत्वपूर्ण हो सकते हैं।
कुछ results equivocal हो सकते हैं और अतिरिक्त testing की जरूरत पड़ सकती है।
IHC में 0, 1+, 2+ और 3+ क्या होते हैं?
कुछ biomarkers, जैसे HER2, के लिए IHC में 0 से 3+ scores दिख सकते हैं। HER2 IHC 2+ result को कई settings में ISH testing से clarify किया जाता है। इसलिए score के साथ interpretation भी पढ़ें।
Cancer Grade क्या बताता है?
Cancer Grade बताता है कि cancer cells microscope में सामान्य cells से कितनी अलग दिखाई देती हैं और वे कितनी differentiated हैं। Grade pathologist निर्धारित करता है और grading system cancer type के अनुसार बदलता है। कई tumors में Grade 1 से Grade 4 तक categories इस्तेमाल होती हैं।
“Well differentiated” cells अपने मूल tissue जैसी अधिक दिख सकती हैं, जबकि “poorly differentiated” cells अधिक abnormal हो सकती हैं। Grade और Stage अलग हैं: Grade microscopic appearance बताता है, जबकि Stage cancer की extent या spread बताता है। इसलिए Grade 3 का अर्थ Stage 3 नहीं है।
Cancer Stage क्या है? TNM को आसान भाषा में समझें
Cancer Stage बीमारी के शरीर में फैलाव या extent को बताता है। कई solid cancers में TNM system उपयोग होता है: T primary tumor की size या local extent, N nearby lymph nodes और M distant metastasis को दर्शाता है। Stage group और staging rules cancer type के अनुसार अलग हो सकते हैं।
Biopsy में कुछ staging information मिल सकती है, लेकिन पूरा stage केवल biopsy से हमेशा तय नहीं होता। Doctors imaging, examination और जरूरत पड़ने पर surgical findings के साथ देखते हैं।
Cancer type, stage और tumor location के अनुसार रेडिएशन थेरेपी treatment plan का हिस्सा हो सकती है। Radiation cancer को control करने, tumor को shrink करने या कुछ स्थितियों में symptoms कम करने के लिए उपयोग की जा सकती है।
Biomarker क्या है और यह treatment में क्यों महत्वपूर्ण है?
Biomarker cancer की biological characteristic बताने वाला measurable feature हो सकता है, जैसे protein, gene change या molecular alteration। Biomarker testing से डॉक्टर यह जान सकते हैं कि cancer में ऐसा target मौजूद है या नहीं जिस पर specific treatment काम कर सकता है।
कुछ biomarkers targeted therapy या immunotherapy चुनने में मदद कर सकते हैं। उदाहरण के लिए, कुछ lung cancers में EGFR alterations treatment selection को प्रभावित कर सकते हैं। हर patient को हर biomarker test की जरूरत नहीं होती; test cancer type, stage और clinical context के अनुसार चुना जाता है।

कौन-सी गलतियां नहीं करनी चाहिए?
“Positive”, “negative”, “high grade” या “Stage III” जैसे किसी एक result को देखकर treatment या outcome का अनुमान नहीं लगाना चाहिए। “pending”, “equivocal”, “indeterminate” या “insufficient tissue” जैसे शब्द further testing की जरूरत का संकेत हो सकते हैं।
Consultation में पूछें: मेरी final diagnosis क्या है? मेरी stage किन reports पर आधारित है? कौन-से biomarkers treatment decision के लिए relevant हैं?
यदि diagnosis जटिल हो या results में uncertainty हो, तो specialist pathology review या second opinion के बारे में treating doctor से चर्चा की जा सकती है। Pathology, imaging और oncology assessment को साथ समझना अधिक उपयोगी होता है।
